Membership is not health insurance. Keep your insurance for hospital care, specialists, imaging, emergencies and prescriptions.

Direct primary care memberships are an HSA-eligible expense under federal rules that began in 2026, up to $150 a month for one person or $300 for a membership covering more than one. Check with your accountant if you have questions.

How a membership works

  1. Step 1

    Talk with Dr. Witt.

Start with a free 15-minute call about what you need and whether we're a fit. Call us or ask us to call you.

  2. Step 2

    Join.

If it's a fit, we email you the membership agreement when enrollment opens in November 2026. You sign it under Arizona's direct primary care law, choose monthly or yearly, and confirm you're not enrolled in Medicare.

  3. Step 3

    Your first visit.

An hour with Dr. Witt to go over your history and medications, and to talk about what you want from your health care.

  4. Step 4

    Day to day.

Message or call the practice line. Dr. Witt calls you back the same day. Same- or next-day appointments on clinic days, evening appointments on alternate weeks.

What being a member looks like

Your membership covers what happens with us: visits, messages and your annual physical. Care from anyone else still goes through your health insurance, which is why we ask members to keep it.

Lab work
Whether performed in-office or externally, lab work is billed to your insurance independently by the provider and not included in your membership.
Imaging
X-rays, ultrasounds, CT and MRI are done at an imaging center. We order them and go over the results with you. The imaging center bills your insurance.
Specialists
We refer you, send your records and talk with the specialist. Your insurance pays for the specialist visit.
Prescriptions
Filled at your pharmacy and paid through your insurance. If you're paying cash, we'll help you find a low price.
Hospital and emergencies
Call 911 in an emergency. If you're admitted, Dr. Witt talks with your hospital team and sees you within days of discharge. Your insurance covers the hospital stay.

Who can join

  • Adults 18 and over
  • Not enrolled in any part of Medicare (Part A, Part B or a Medicare Advantage plan). If you're on Medicare, see our Medicare page
  • Willing to keep health insurance for the big things. A high-deductible plan pairs well with membership

Money and terms

Is this health insurance?

No. Membership covers primary care from us, under Arizona's direct primary care law. Keep health insurance for hospital stays, specialists, imaging, emergencies and prescriptions. A high-deductible plan pairs well with membership.

More questions and answers

Do you take insurance?

We don't bill insurance for anything in your membership. That's how we keep one flat price and spend our time on you instead of paperwork. Your insurance still works for the care we refer you to.

More questions and answers

Can I pay with my HSA?

Yes. Under federal rules that began in 2026, direct primary care memberships are an HSA-eligible expense, up to $150 a month for one person or $300 for a membership covering more than one, and having one doesn't stop you contributing to an HSA. Check with your accountant if you have questions about your own situation.

More questions and answers

How much does it cost?

$120 a month for one adult, or $1,368 a year if you pay yearly (5% off). Each additional family member at your address is another 5% off. Our first 100 members pay a founding price of $100 a month or $1,140 a year, locked for three years. No enrollment fee. Pricing.

More questions and answers

Can I cancel?

Yes, any time, with 30 days' written notice. Unused prepaid months are refunded. Arizona law requires this and we'd do it anyway.

More questions and answers

Are there any other fees?

No enrollment fee and no per-visit fees. Whether performed in-office or externally, lab work is billed to your insurance independently by the provider and not included in your membership. Imaging and prescriptions are billed by whoever provides them, and we help you find low cash prices if you need them.

More questions and answers

Can my spouse or partner join?

Yes. Each additional family member at your address is 5% off: $114 a month, or $95 at the founding price. Everyone who joins needs to be 18 or over and not enrolled in Medicare.
